How to refer

You can refer people to green social prescribing through one of Surrey Heartlands' social prescribing services. Social prescribing addresses health inequalities and the wider determinants of health by connecting people to sources of help and support within local communities. It takes a holistic 'whole-person' approach to health and social care asking 'What matters to your?' and not 'What's the matter with you?'
